If your Administrator has changed for your practice, please provide us with the following information:

  • Current Administrator's full name
  • New Administrator's full name
  • New Administrator's e-mail address
  • Healthplan name
  • Practice address
  • Practice Tax ID

We will create an account for the new administrator with this information and send the login details to his/her e-mail address.
