If your Administrator has changed for your practice, please provide us with the following information:
- Current Administrator's full name
- New Administrator's full name
- New Administrator's e-mail address
- Healthplan name
- Practice address
- Practice Tax ID
We will create an account for the new administrator with this information and send the login details to his/her e-mail address.
